Abstract
We report a 39-year-old man with recurrent herpes simplex virus (HSV) encephalitis whose brain MRI revealed bilateral high signal intensity of the hippocampus despite administration of intravenous acyclovir for presumed herpes encephalitis. This case not only demonstrated unusual MRI findings for recurrent HSV encephalitis, but it also revealed that HSV encephalitis can relapse despite intravenous antiviral treatment. Therefore, close observation or sufficient duration of treatment is needed for patients with HSV encephalitis.
